Welcome to Emporia State University Athletics. We are pleased and excited that you have decided to join the ESU Athletic community!
While you are a participant here at ESU our Sports Medicine staff is dedicated to giving the best medical care available. In order to do this we ask that you complete all the necessary paperwork, including supporting documents, in a timely manner.Â
All forms will be completed online through Front Rush/ACS. Any additional documents can be sent to ESU Sports Medicine using the contact information listed below or turned in by the student-athlete at their physical examination.Â
1.   All Student-Athletes must have medical insurance that covers athletic injuries prior to participation at ESU. If you currently do not have insurance that covers athletics, contact Emporia State University Health Services to purchase insurance.
2.   When completing your Medical History, additional information is necessary if the following applies to you:
- If you have had an injury within the LAST YEAR that required an orthopedic evaluation or surgical intervention, we ask that you bring medical records pertaining to this injury for our medical record stating your clearance or diagnosis of the injury.Â
- If you have a known heart condition that has been evaluated by a cardiologist, please bring records stating what condition you have been diagnosed with.Â
- If you have been diagnosed with ADD/ADHD we ask that you contact us at the address and phone number indicating that you have been diagnosed. We ask this because the NCAA requires documentation of ADD/ADHD diagnoses in the situation that an athlete must take a randomized drug test.
3.   Baseline concussion testing will be completed on campus during physicals. Information on additional online testing will be emailed to student-athletes and should be completed prior to their arrival on campus.

4. Returning Athletes: Complete all forms on Front Rush as instructed. Medical updates will be completed on campus with an athletic trainer after July 1st each year.

Supplying ESU Sports Medicine with these documents greatly improves our ability to properly care for you, the student athlete. We ask that these documents be completed on Front Rush/ACS and any additional documents be submitted to ESU Sports Medicine prior to your first semester at ESU. Failure to do so will result in the disqualification of athletic participation for the student athlete until documentation is received by ESU Sports Medicine.
